Coming Soon: Continuity of Care Documents

13 May 2016

The majority of healthcare providers have implemented certified electronic health record technology (EHR) over the past five years. The goal in automating health record technology is to enable healthcare providers with the ability to share a patient’s health records from doctor to doctor regardless of the geographic location. This way each doctor involved with the care of a patient will have access to the most current health information available for that patient. Having the latest health information available will eliminate duplicate tests and work to reduce the overall cost of healthcare.

To ensure each healthcare provider has access to the latest clinical information available for each patient, the Federal Government has mandated all doctors to provide a (CCD) Continuity of Care document to each patient for each patient visit. Currently, a doctor has the option of printing a CCD document or providing the CCD document electronically. After each visit to the doctor, the doctor has 3 days to provide you with a CCD document representing that visit. By the end of the 4th quarter of 2012, ViewMyHealthRecords.com will release our next version of online health record software that will provide “real time” access to your CCD documents.

How many times have you showed up for an appointment and been asked to complete paperwork designed to list all your past medical history? These forms require you to list your allergies, current medications, recent past medications and medical diagnosis. By having access to your latest CCD documents, you can easily provide any healthcare provider with your most recent health information on file. This would eliminate the need for you to fill out paper medical history forms and use technology available by ViewMyHealthRecords.com to save both you and your doctor time.

Beginning in 2014, CCD documents will be engineered to provide test results too. This would include lab results and interpretations from diagnostic studies such as x-rays, MRI’s and CT scans. If your doctor is not providing online access to your health records then recommend that they contact ViewMyHealthRecords.com today.
